Health Forms 2021-22

 
 
Health forms for the 2021-2022 school year may be accessed from our website or by clicking here. Please fill out your part of the forms and have your pediatrician complete and sign their portion.
 
Every student must have a current health form on record in order to participate in gym. As in the past, we will use a "sliding scale" of due dates, so that if your child's annual exam took place in December 2020, the new one will not be due until December 2021. Please remember that it may take time to obtain the appointment date and time of your choice in your pediatrician's office, so please plan accordingly. If a current health form is not on file in the nurse's office, your child will not be able to participate in gym.
 
Students who are new to Moriah must have their immunization records in place by the time school begins in September. Those students who are less than 5 years of age must have a record of a flu vaccine for the coming season, to be administered no later than December 31, 2021.
 
For those students who will be part of the Moriah sports teams, it is essential that there be a current health form on record. There will be no exceptions, so again, please be cognizant of this and be sure to set up your child's appointment in a timely manner. The Clearance form, a New Jersey Health Department requirement, also has to be on file. It is part of the Health Forms packet and can be found on the school website. These will be collected as the updated health forms are submitted. Again, there will be no exceptions.
 
The eye exam form may be filled out by your pediatrician, optometrist or ophthalmologist. Eye exams required annually, so please have the forms filled out and returned- especially for those students who are currently wearing glasses or contact lenses.
 
It is expected that every student in Moriah will be fully immunized according to the standards set by the American Academy of Pediatrics and the Health Department of the State of New Jersey. No students will be permitted to attend school until all of the requirements are met.
